This is a contract role for a Care Manager role at WatchRx, Inc. The Care Manager will be responsible for communicating with patients making sure patients follow the care plans prescribed by physicians, monitoring patients vitals and checking their general health. This position involves working in person at clinics to ensure effective collaboration.
Job responsibilities of a care manager:
- Manage and implement care plans for patients enrolled in CCM/PCM and RPM, under the direction of healthcare providers.
- Track and analyze health data from remote monitoring devices to detect trends and changes, and escalate issues as needed according to established protocols by the provider.
- Conduct routine patient check-ins, either in the clinic or remotely, to assess their health and address any issues.
- Work closely with physicians and healthcare providers to address care gaps and improve patient outcomes.
- Educate patients on provider-recommended self-management strategies and lifestyle changes.
- Maintain accurate and timely documentation of patient interactions and updates in the EMR system.
- Coordinate follow-up appointments and referrals to other healthcare services as required.
- Assist patients in navigating healthcare services and obtaining necessary resources for their care.
- Identify potential health risks and escalate concerns to providers for timely intervention.
- Foster patient engagement and adherence to their care plans to improve long-term health outcomes.
Qualifications
- Medical Assisting and Patient Care skills
- Knowledge of Medical Terminology
- Experience in a Medical Office setting
- Proficiency in managing Medical Records
- Strong communication and interpersonal skills
- Ability to work independently and in a team
- Certification as a Medical Assistant or Licensed Practical Nurse
- Experience with electronic health records (EHR) systems
